While some patients hesitate to apply to clinics with outdated information such as "diabetics cannot receive implants", others think that their existing chronic diseases will completely eliminate implant success. However, international studies published in the last five years reveal that with the right patient selection, optimal metabolic control and personalized treatment protocols, implant treatment can be applied with high success rates in individuals with diabetes.

As Private Misyon Oral and Dental Health Polyclinic, we aim to answer this question in the light of current scientific evidence and in a completely transparent manner. In this article, we will elaborate on the relationship between diabetes and implants not only at the level of "yes" or "no", but also under which conditions yes, under which conditions it should be expected, the basic parameters that determine success and Misyondent's approach to this sensitive patient group.

The Real Impact of Diabetes on Implant Success: Outdated Paradigms and New Data

For many years in the medical and dental literature, diabetes was considered a relative contraindication for implant treatment. This approach was based on the fact that hyperglycemia delays wound healing, increases the risk of infection and negatively affects bone metabolism. However, large-scale systematic reviews and meta-analyses conducted after 2020 have shown that diabetes is not the sole determinant of implant success; the main determinant is level of glycemic control that he was.

A systematic review published in 2024 evaluating implant survival rates in patients with type 2 diabetes, HbA1c value below %8 The study showed that the one-year implant survival rate in patients with well-controlled diabetes ranged from .1 to .3, while at five-year follow-up the rate ranged from .3 to .1. These figures almost exactly match the implant success rates of healthy non-diabetic individuals.

In contrast, the situation is different in patients with HbA1c ≥ %8 with poor glycemic control. In another study from 2025, it was reported that the early implant failure rate in individuals with uncontrolled diabetes increased to %6-8, while the incidence of peri-implantitis reached very high values of -31. This data clearly demonstrates that uncontrolled diabetes poses a risk, contrary to the old belief that diabetes directly prevents implant success.

At this point, a critical distinction must be made: Having diabetes is not an obstacle to implant treatment. The obstacle is uncontrolled blood sugar levels.

The Biological Basis for Success: Bone Healing and Osseointegration Process

Osseointegration, which forms the basis of implant treatment, is the establishment of a direct structural and functional connection between living bone tissue and the implant surface. This process depends on the flawless functioning of complex cellular mechanisms. Diabetes, especially when uncontrolled, affects these mechanisms in various ways.

Chronic hyperglycemia leads to the accumulation of AGEs, advanced glycation end products. These molecules suppress osteoblast activity in the bone matrix, slowing new bone formation. At the same time, they increase osteoclast activity by triggering the release of inflammatory cytokines, accelerating bone destruction. A 2025 review emphasizes that diabetes reduces peri-implant vascularization through microangiopathy and impairs oxygen transport to the tissue during the healing period.

All of these biological processes have led to the development of implant therapy in individuals with diabetes. more careful planning and multidisciplinary approach mandatory. For us, each patient with diabetes is an individual with a different metabolic profile, on different medications and at different risks of complications. Therefore, the first step in treatment planning is to know the patient.

We start our treatment process with a complete inventory of the patient's current systemic condition. The patient's diabetes type, duration of diagnosis, HbA1c value, antidiabetic drugs used, frequency of hypoglycemia attacks and diabetes-related complications (nephropathy, neuropathy, retinopathy, cardiovascular disease) are meticulously recorded. In line with this information, the patient's decision for implant treatment is made. risk profile and the treatment decision is taken transparently, together with the patient.

Another element that makes Misyondent different, our strong reference network is our capacity to work in an integrated manner. When necessary, we consult our patients with endocrinology, internal medicine or cardiology specialists to ensure optimal metabolic control before surgery. This multidisciplinary approach is one of the cornerstones of our treatment success.

In the surgical stage, we have developed a special program for our patients with diabetes. non-traumatic surgical protocol we minimize tissue damage and accelerate the healing process. All the materials we use during surgery are selected taking into account the changing healing capacity of the patient with diabetes. We prefer implants with bioactive surface properties and biomimetic materials that accelerate healing in cases where bone grafting is required.

After treatment, for our implant patients with diabetes special maintenance program we create. Within the scope of this program, the patient is called for follow-up more frequently than healthy individuals (every 3-4 months). At each visit, peri-implant tissue health, probing depths, bleeding index and marginal bone level are evaluated. Any early signs of inflammation are treated aggressively to prevent the development of peri-implantitis.

One of the most frequent indicators of satisfaction that our patients express is that they feel more comfortable at Misyondent. only one case not as individual to feel understood and safe.
