Our patient named Nilgün E. applied to our clinic with the aim of obtaining a more aesthetic, more balanced smile that is compatible with her facial structure. In the first evaluation, it was seen that our patient expected a more natural appearance, especially in terms of tooth form, color harmony and smile line in the anterior region.

In this process, our aim is not only to change the color of the teeth; our patient's lip structure, smile line, gum appearance, facial expression and existing dental tissues are evaluated together. a personalized, natural and balanced smile design was to create.

A treatment plan was created for our patient after clinical examination, intraoral photo evaluation and panoramic x-ray examination.

    Applied Treatments

In this case, the following procedures were applied to our patient:

    • 19 pieces of lamina veneer
    • 1 zirconium crown
    • 1 root canal treatment
    • 1 night record

Each tooth was evaluated individually when creating the treatment plan. In the aesthetic area, a more natural, light-transparent and compatible appearance with the tooth tissue was targeted. lamina coating was preferred. However, since the existing structure and support needs are different in a tooth, it was preferred to zirconium coating planned.

The tooth that required root canal treatment was included in the treatment process, aiming to make a healthier planning both aesthetically and functionally. After the treatment was completed, in order to support the long-lasting protection of the restorations made, our patient night record Prepared.

Which complaints did our patient present to our clinic?

When Nilgün E. came to our clinic, her teeth in the anterior region did not meet her aesthetic expectations in terms of color, form and alignment. The appearance of the teeth during the smile affected our patient's facial expression and she expected a brighter, more organized, more natural smile.

As seen in the initial photograph, there were color differences, form irregularities and areas in the smile line that could be aesthetically improved. For this reason, not only individual teeth were considered in the treatment planning, but the whole smile as a whole.

At this point, our patient's expectation was this:

    • A lighter but not artificial tooth color
    • Smoother and more harmonious tooth forms
    • A smile line in harmony with the lip structure
    • A natural-looking, understated aesthetic result
    • A comfortable and functional treatment result in everyday use

    Initial Examination and X-ray Evaluation

Before starting the treatment, panoramic x-ray and intraoral examination of our patient were evaluated. At this stage, the root structures of the teeth, existing restorations, tooth support tissues and general oral health were examined.

In aesthetic dentistry, it is not correct to make a decision only by looking at the external appearance. Especially before restorative procedures such as lamina or zirconium, factors such as the infrastructure of the teeth, the need for root canal treatment, bite relationship and clenching habits should be evaluated.

The treatment plan of our patient was created with this principle. First of all, the existing oral and dental health was evaluated, then restorative planning was made in accordance with aesthetic expectations.

    Why 19 Lamina Veneers Planned?

Lamina veneers are one of the most preferred restorations to achieve a natural appearance, especially for aesthetic expectations in the anterior region. In this case, it was aimed to achieve a more natural light transmission, more elegant tooth forms and a smile in harmony with facial expression.

The main reason for planning 19 lamina veneers was to harmonize the teeth visible in the smile. Because changing only a few teeth can disrupt the integrity in terms of color and form in some cases. For this reason, a more balanced result was aimed by evaluating the teeth visible in the smile line together.

With lamina veneers, the shade, length, width, surface form and position of the teeth in the smile were arranged in a more aesthetic integrity.

Why Zirconium Veneers on 1 Tooth?

The needs of each tooth may not be the same. While lamina veneers may be sufficient for some teeth, some teeth may require a restoration that provides more support.

In our patient, zirconium crown was preferred for that area by evaluating the existing structure of a tooth, its durability need and its position in the treatment plan.

Zirconium crown is an option that can be preferred in cases where aesthetic appearance, durability and all-round support of tooth tissue are required. In this case, it was planned as a more suitable solution for the relevant tooth without disrupting the integrity of the overall smile design.

Why was root canal treatment necessary?

During the treatment process, the need for root canal treatment was detected in a tooth. It is very important to prepare the tooth on a healthy foundation before starting aesthetic restorations.

Root canal treatment is a treatment applied when there is a problem in the internal tissue of the tooth. The aim is to keep the tooth in the mouth, to control the risk of infection or pain and to ensure that the restoration to be made on it is planned on a healthier basis.

Therefore, root canal treatment was not considered as a part of the aesthetic process, but as a basic procedure that supports oral health before restoration.

What was Considered in Smile Design?

In this case, the treatment planning was not only aimed at "white teeth". The main goal was to create a smile that looks natural and in harmony with our patient's facial expression.

The following details were therefore considered together:

    • Color of teeth
    • Length and width of the teeth
    • Relationship of teeth to lips
    • The amount of teeth visible during a smile
    • Gum level
    • Centerline harmony
    • Face shape and mimic structure
    • Lower and upper jaw closure relationship
    • Teeth clenching or habits that may strain restorations

As a result, our patient's smile became brighter, more organized and more aesthetic. However, when planning this appearance, we aimed for a result in accordance with the natural aesthetic perception, rather than an artificial whiteness.

Why was a night plate given after treatment?

Post-treatment preventive measures are important for the long-term healthy use of aesthetic restorations such as lamina and zirconium. For this reason, a night plate was prepared for our patient.

Night plaque helps to protect restorations, especially in patients who tend to clench or grind their teeth. By helping to reduce uncontrolled forces on the teeth, it can help to protect both the natural tooth tissues and the veneers.

In this case, a night plate was planned to help maintain the treatment outcome in the long term.

What kind of change was achieved as a result of the treatment?

When the treatment was completed, a significant aesthetic transformation was achieved in our patient's smile. Teeth, which initially appeared more irregular in terms of color, form and smile line; at the end of the treatment, they had a brighter, more balanced and more harmonious appearance.

As can be seen in the final photo, tooth forms have become more regular, color harmony has been improved and the smile line has gained a more aesthetic integrity.

Our most important goal in this process was to achieve a natural-looking result suitable for our patient's facial structure. After the treatment, our patient's smile became more vibrant, cleaner and more aesthetic.

What Does This Case Show Us?

This treatment process is a good example of how aesthetic dental treatments are not just about lightening the color of teeth.

    For a successful smile design:

    • Detailed examination should be done first
    • X-ray evaluation should not be neglected
    • Each tooth should be planned separately
    • If necessary, infrastructure procedures such as root canal treatment should be completed
    • Different restorations such as lamina and zirconium can be used together in the same case
    • Supportive applications such as protective night plates should be considered after treatment

The result should be in harmony with the patient's facial structure and expectations; therefore, a standardized treatment plan cannot be applied to every patient. The plan prepared for Nilgün E. was created individually by evaluating our patient's existing tooth structure, aesthetic expectation and intraoral condition.

    Frequently Asked Questions

Is lamina veneer applied in every aesthetic smile design?

No, it is not. Lamina veneers may not be suitable for every patient. The current condition of the teeth, enamel structure, bite relationship, clenching habits and aesthetic expectations should be evaluated together. While lamina may be sufficient for some patients, zirconium, e-max, orthodontics, gingivoplasty or other treatments may be required for others.

Can lamina veneer and zirconium veneer be applied at the same time?

Yes, in some cases different restorations can be applied to different teeth. This depends entirely on the needs of the tooth. In this treatment plan, lamina veneers were applied to 19 teeth, while zirconium veneers were preferred for one tooth. The aim is to maintain overall smile harmony by determining the most appropriate type of restoration for each tooth.

Does root canal treatment prevent aesthetic treatment?

No. If there is a tooth that needs root canal treatment, this treatment may need to be completed before aesthetic restoration. After root canal treatment, appropriate restoration can be planned according to the condition of the tooth. The important thing is to perform aesthetic procedures on a healthy foundation.

    Why is night plaque important?

Night plaque can help protect restorations, especially in patients with a tendency to clench or grind their teeth. For long-lasting use of lamina and zirconium veneers, it is important for the patient to pay attention to oral care and follow the preventive measures recommended by the dentist.

Will this result be the same for everyone?

No. Each patient's tooth structure, jaw relationship, gum level, facial shape and aesthetic expectations are different. Therefore, the treatment plan and the result may vary from person to person. This case shows the personalized planning for Nilgün E.
